[...]. The aim and objectives of the study. The aim of the study was to reveal the processes of creating and implementing integrated team-based home care as a new service, and to evaluate the effectiveness of the service for the patients in neeed of constant care from the viewoint of the care receivers and the care providers. Objectives: 1. To reveal the processes of creating and implementing integral home care provided by mobile teams as an innovative service, and to evaluate the eeffectiveness of the service; 2. To assess the change in the professional interaction between the nurses and the social workers in the mobile integrated care teams and the interaction of the teams wih the existing structures of the health care and the social security from the viewoint of the care providers; 3. To assess the quality and effectiveness of the integral home care from the viewpoint of the care providers and the care receivers; 4.
